Just filled out one of these for our Galaxie its a little daunting but Trust me most of it you leave blank ... gave very few details of actual build date and it came back in less than a week with a nice age related plate no probs .

one bit i dont know is where it says "vehicle body type"
the example shows Coupe, because i am guessing it was a coupe.
what does anyones say that is a fastback. is that still a coupe or does it say something different on your V5

Just gone through the same thing--couldnt answer half the questions, so rang dvla, and the woman said oh you dont have to answer all the questions, but you MUST answer x, x, x etc--I suppose the unused questions are to satisfy all the other scenarios. But it is quite daunting when you first look at it.
